One hosting domain, many customer forms

Rooks hosts forms for organizations that use the Rooks platform. The organization that created a form controls its content and decides who should receive its link.

• Intake and registration. Organizations can collect details for onboarding, registration, scheduling, or other operational workflows.
• Applications and questionnaires. Customer-created forms may support applications, assessments, surveys, and structured questionnaires.
• Other data collection. The same form-hosting model supports many kinds of information gathering, depending on the customer’s needs.

Shared form-hosting infrastructure, customer-owned content

Rooks provides form hosting infrastructure for forms created and published through its platform. A form on a customer-specific onrooks.com subdomain is administered by the Rooks customer identified in that form.

Rooks does not represent that every customer-created form is reviewed, endorsed, or appropriate for every visitor. If a form asks for information, consider why it was sent to you and who is requesting it.

There is no form directory here. This page explains the domain; it does not list customer forms, submissions, or customer names.
